-Current lease agreement from a leasing agent/apartment                              

-If you are leasing from an individual, you must have a copy                           

of the lease and current utility bill

Required for students transferring from outside Fayette County:

__Birth Certificate                                                                                                                    

__Social Securtiy Card                                                                                                             

__Current Kentucky Immunization Certificate                                                                    

__Previous school year report card/transcript                                                                   

 

Required for students transferring from outside the United States:

__Passport                                                                                                                                 

__TB Skin Test with results for those students entering                                                    

from countries w/ endemic tuberculosis. (Asia, Afterica, the Middle East & Latin America)
